1. 3 months is for risk control. The longer the better. Two months is probably ok in my opinion.
2. New I-864 no need for notary.
3. All concurrent filing should go to Chicago lockbox. I think it is listed on I-485 instructions.
4. Should be I-94 date.
5. That's probably fine. -- Either way not a big deal.
6. Fill G325 electronically -- once you do the 1 page, Adobe SW fills up the rest for you.
7. Copy is fine.
