easy case

if you are LC(Perm)based, you can extend your h-1 year by year after it's been pending for 1 year
if NIW, EB1A ,can extend your h-1 year by year after 140 has been pending for 1 year

if 140 is approved, you can extend for 3-year after your h1 expire(just apply for extension before it expires)
according to current PERM pace, your 140 should get approved within a year from (perm to approved 140)
