Questions on Medical Exam

I noticed that some guys said we can submit physical examination results later. But the I485 says that we need to fiel it together with I485. I am filing I140 and I485 concurrently, what should i do? Should I file everything required on 485 except medical examination?

I called several doctors's offices, seems to be quite expensive? Around 200-250 dollars.

How you guys fix this problem.
