two questions about G-325, please help!

1. My father passed away, can I live the father's name, birthday blank?
2. on my birth certificate, the spell of my mom's name is wrong. SHould I put her right name on the form? I asked my family to do it again. But we can't get it in short time. or Can I explain it using the mother-daughter certificate where my mom's name is right?
