485 wom summons question

i'm working my 485 wom case, but need help filling the summons. here are my 2 questions:

1. on the first page, how many days should i give the defendants after service of the summons? 60 days?

2. do i need to fill anything on the second page titled "RETURN OF SERVICE"?
