请教:I-131 questions

1. Can you file I-131 with I-485 in the same envelope?
2. What to fill in at "Class of Admission"?
3. For "Application type", if it's H4 filing 485, should "d" be checked?
4. Usually should you put a date within 3 months as your Date of Intended Departure? If you put December 2007, will is be sent back to wait till September to file?
5. If you only put China as the country you plan to visit but you go to some other countries later, will it be a problem?
