The list of materials for your child are:

1. Copy of your I-140 approve note if you have, otherwise your I-140 receipt instead of approve note. For determine your child is eligible to apply I-485

2. Your child's H4 copy( F1, I-20 or others to determine his current stauts)

3. Your child's birth certificate to indicate you and your child's relationship.

4. Copy of your child's last entry visa from his passport to indicate he/she is legal entry to USA.

5. Photos of your child

6. I-485 application form.

7. Check made to US homeland security.

9. Child's passport copy

10. A sealed medical examination report

11. AP application form for child if you want

12. EAD application form for your baby if you want
