1. You better sub mit all pages of passport, because the officer can see your whole records of entering and exiting US. if your officer has time to look over or not that's his problem, but if you provide or not, that's your problem.
2. Better sub mit all approval notice so that INS can establish your legal status in US all these years. If you don't provide, how can INS know?
3. You can take your photos yourself, but follow the INS instruction.

Bottom line, sub mit as many documents as possible, because you don't want to get REF. Who knows what INS needs. Hours more preparation may save months of GC approval.
