Place of Last Entry Into the USA (city/state)
Date of last Arrival

My home address is Seattle (and from China, I landed at Seattle too), But I visited Canada for serveral times by driving. The first time I visited Canada and when I came back, the INS replace my old I-94 with a new I-94 at port of Blaine, WA. And after that I never change I-94 and the immigration officer only simple asked me why to visit Canada

So this field, I should fill with Blaine/WA instead of Seattle/WA

And date of last arrival should be the first time that I came back from Canada (the date I got my new I-94)
