Incorrect number on I-94 Card

Hello there,

I got my H4 last year. When I filed AR-11 form for address last week I found that the number on my I-94 card was not the came as one on my old I-94 card. I went to Toronto last year and my old I-94 card was kept by INS saying I had to turn it in.. What can I do to correct it? What could be the consenquence? If anyone can direct me in this matter I really appreciate.
