Immunization record for my kid--to land Canada

Dear all, I need your kind help. My question is:

The immunication record---for the applicant under 16 years of age---need to be completed and certified by a qualified health professional. Who can be the quealified health professional? My kid's school nurse? My son's family doctor? or my county's Immunization office?
