I have the exact same situation (3 years H1, expired date 9/30/07, and approved I-140) with you. Today law office of my company let me mail my wife, my son, and my current I-94s, last two payrolls,my updated resume, my home phone number, and my home address to them to extend my H1 and my family's H4.
