This package includes:
1, Original Form I-94 of both 2 applicants;
2, a check of $200.00 for application;
3, filed Form I-539;
4, a copy of the filing receipt noting that my Form I-129 was pending with the USCIS;
5, a copy of my (employee‘s) Form I-94 and approval notice showing that I have been granted statues to the period requested on the application;
6, Evidence of relationship (for my wife, Marriage Notarization; for my son, Birth Notarization).(注:复印件).
