I-94 and permanent resident

(1)Should permanent resident fill I-94 arrival-departure record when returning to U.S? Who can let me know with your experience?

(2) How does INS know when permanent resident leaves U.S., since LPR does not need to submit I-94 to airliner at the time of leaving?
