XiaoBaiTu/Radiology please advise: H-1B Change of Status

My wife got her H-1B petition appproved last year. She was still in F1-OPT so she chose to change status later from out of US. But now she changed her mind and want to change status inside US. Can that be done? Her OPT has 4 months left.
BTW, her immigration attorney said "he's not sured how to do it..." Does it show this guy is incompetent?
