Queations on J-1 waiver. Help me please

I am a postdoc in J-1. I want to created a J-1 waiver online.I have three queations now.

First, which items should I chose.

1 No Objection statement from the home government
2 State Health Agency Request
3 Request by an Interested (U.S.) Government Agency, or IGA
4 Exceptional hardship to a United States citizen (or permanent resident) spouse or child of exchange visitor
5 Persecution

Second, if I finished and print it out and found some mistake, can I create one more?

Third, which items should I chose on ....funds, funds from your homecountry gov... Should I chose no or yes?
