urgent -- question about I-129 (Xiaobaitu ...)

I got a job and doing H1 transfer. I have some questions about I-129.

In part 2, question 2 Basis for Classification, should it be "Change of employer"?

In part 2, question 5 Requested Action, should it be "Amend the stay of the person(s) since they now hold this status."?
