which is ist step for waiver?

我查了DEPT. OF STATE and 纽约总领馆有关WAIVER 的PROCEDURE, 不尽明了. Which is first step,向美国国务院申请豁免编号（Case Number）or 领取《留学人员豁免申请表》? How to apply case number? Need I fill out DS-3035 for this case number?

Moreover, Does this application include J-2 ? Yes or no?
