I am sure you are the employement based GC or citizen. So you need to use I-864 instead of I-864EZ. Also, the form yu see in the USCIS can be used right now. It doesn't expire although the dates show in the top. But you did not read: 1/15/06. 11/05/01, 9/26/00, 1/21/98 also acceptable through 10/19/06.
