Questions about F1/OPT to H4

I am now working under F1/OPT which will expire on 12/30/2006. My current job will finish in middle of Oct. I need to transfer to H4 after my f1/OPT expires. I am wondering:

1. When should I the application for the tranfer?

2. Could I ask for the start date of H4? Is it from the date of the approval or date of the F1 expires?

3. Do I need to the original I-94 or just the copy?
