medical exam

I-140/I-485 concurrent filed June 2005
I-140 just approved
Just did medical exam
Someone told me I should send medical exam now. someone said should send after receiving a RFE. What if the medical exam expires when the RFE arrives?
Should I send it now or wait for the RFE???????????
