Ø The signed Form I-539;
Ø Legible copies of EVERY family member’s passport information including biographical information page, all U.S. visa pages and I-94 form (front and back);
Ø Evidence to show the relationship between the H-1B Applicant and family members; AND
Ø Check/money order in the sum of $140 to cover the filing fee for family, payable to Department Of Homeland Security.
