WR-702 Processing Sheet for Form I-485 necessary?

I requested a packet of AOS. In front of the form I-485 there is a sheet "Processing Sheet for Form I-485, WR-702" requesting personal information.

My question is: Do I need to file it? I will file my I-485 with fillable form. Can I type a processing sheet for I-485 in word format?
