Sunnyvale there is one at Remington Ave and El Camino Real. Cannot remember the Dr. name, but address at
500 remington ave (str?), sunnyvale. you can check the website and find that one.

only about 150 - 250 depends on what you need to do. very fast, an old lady doc.
