How can I be a resident doctor without green card?求教小白兔

My I-140 is approved(EB1) and I filed my 458. I am applying for resident doctor. How can I work as a resident without greencard, H1B or J1? Can I apply for a paperwork(because of my approvement of I140(EB1)) that let me shift my job from basic medical science to clinical work and legally work for a hospital?
