You can go to any doctor certified by USCIS to get the letter first and mail them together to USCIS. Or you can send the other forms first to USCIS. Then USCIS will send you a letter to give you a doctor list. You can choose one of them and see the doctor and get the letter. Either way is fine. It is said the mailing them at the same time is quicker.
