1. Call 1-800-375-5283, there will be a choice for medical doctor information after several choices. Input your zip code, you will hear a recording with the docter's information

2. I just made an appointment, but I couldnt' find out how long the check is valid. If you are not sure, wait until a couple of months before you are ready to submit your application.
