How specific is the same field requirement for NIW?

My friend has an NIW based 140 approved. He is about to graduate and work as a postdoc. He is in the biomedical research field, which is so broad, and wonders if the future position must be exactly the same or similar. How speciifc is the same field requirement. e.g. 1. from colon cancer to colon cancer; 2. from colon cancer to another cancers, or 3.like lung cancer or leukemia; or from one disease like cancer to another disease like diabetes? or simply in the biomedical research field other than chemistry or computer.
