I bought a "do it yourself" kit for 140 and 485. You can do several applications with 485, like AP or work permission for your spouse.

The following is my 485 checklist: (that was 2 years ago and you'd better check by yourself to see if it still fits nowdays.) Please note I didn't include my physical examination documents along with my 485 application. You'd better finish it with your application, or you will get REF form INS later.

1) I-485 filling fees for my family (2 adults and 1 child under 14)

2) I-485 forms for my family member, including my son, my wife and me.

3) Finger print fees (2 adults)

4) My wife’s and my G-325A Forms

5) My wife’s I-765 Form and filling fee

6) A copy of I-140 action notice (copy of I-797)

7) Form I-864, Affidavit of Support, with a Bank statement, and the following:
a, Employment letter (1 original and 4 copies)
b, Tax Return of last 3 years

8) All I-94 copies (2 copies of each person)

9) Photographs (4 of XXX, 2 of XXX and 2 of XXX)

10) The following items of 4 copies for each:
a, My H1 approve notice
b, All IAP-66 Forms and I-612 waiver
c, My wife’s H1 approve notice
d, My wife’s’ INS employment authorization cards copies
e, H4-approval notice for my son

11) Birth Certificate (2 copies for each person)

12) Marriage Certificate (3 copies)
