is income tax return filing required for GC appl.?

can anybody kindly tell me if the income tax return filing is ever required for green card application at any stage? what if i lost a copy of the filing in one of the previous years? what if i have an error in the previous filing that hasn't been corrected yet (due to procrastination, retardness, etc.)? thanks a lot in advance for any reply.
