question about G325, I130.....

Do both of us need to fill out 4 pages of G325? They are repeating information.

In I-130, C14 about the I-94 record, "Date authorized stay expored, or will expire, as shown on Form I-94 or I-95", what should I put in here? I don't see any date on I-94 besides the arrived date....
