I-129W H-1B Data Collection

I am so frustrated to get the whole package of H-1B petition back today. INS needs my company to complete I-129W H-1B Data Collection & Filing Fee Exemption form. Please please anyone who has met the same problem kindly give me some advise. Why there is additional form for me and do I need to pay additional $1000 (not premium precessing fee)? I really appreciate your help.
