help: passport renew

Hi, I want to send my passport for renewal. But after reading the website
introduction from Chinese Embassy in NY, I'm still not sure about the
following questions :

i) Are I-94, I-20 or SSN required ?

ii) For the photocopy of first 7 pages of the passport, which page to start,
the one with my picture ? I noticed there's page # on my passport. But
the 1st page is not the one with my picture. Also what's the photocopy
style : 1 page of passport -> 1 page paper, or
2 pages of passport -> 1 page paper ?

iii) To fill the H-2 form, what's the visa # : the controll # with 14 digits,
or another (red) one with 8 digits ?

Could anyone give me any informtion ? Thanks very much.
