Help! PERM Validity Date

My PERM was approved on March 03, 2006. Because I was so busy last two months, I forgot to send some documents to my lawyer for I-140 filing . And my lawyer didn't even remind me of this.

Does this mean that I will not be able to file I-140 any more? What should I do? I am so mad right now ... at myself and my lawyer.
