Got 2nd letter for ME but need

I received my 2nd letter from Buffalo asking for ME at last. They also asked to take IELTS and said at this time don't need to have interview.
My timeline is:
Apply: Sep. 2002
First letter: Nov. 2002
Second letter: Dec. 2003

Anybody can tell me how to apply for IELTS (Where? How much? And so on)
Do I need to make a reservation for my ME?
