When to file H1B, help please

My opt expires on 10/18/2004, when should I file HIB application?
Can I file it in April, 2004 with a start date 10/1/2004? What if the quota has been reached in April, 2004? Will they reject it, or keep it till 10/1/2004? will they notify me? When will they approve it if they keep it till 10/1/2004? I only have 2 weeks in Oct. 2004, if something goes wrong and I heard that there won't be premium processing in the future, I won't be able to work after 10/18/2004.
I am really confused, any inputs are appreciated.
