Do I need eta 750b forms (2 copies) for I140 NIW?

I am filing I140 NIW. I remember one guy mentioned his petition was returned because of wrong eta 750 B form. Would somebody who submitted I140 NIW recently please give an answer. Do I need ETA 750b (two copies) I140 NIW?
