My mother also came to US with a multi-entry B1 visa. When it came to close to the end of her approved staying period (6 months). We took her to a tour in Vancouver for several days and came back to US. The US immigration officer asked lots of questions and only gave her a new visa with the exact returning date as if the round trip to Vancouver never happened.
This might be only an isolated case though.
