J-1 waiver document needed for I-485 application

Does snybody know which document you need to submit for J-1 waiver when you submit I-485? An official notice from USCIS? or the favorable recommendation letter from IGA application?

Here is my situation: I applied my J-1 waiver through IGA last year. On Dec.1, 2005, a favorable recommendation letter was send to USCIS. Three months passed, I have not received anything from USCIS. What should I do?
