How to re-apply Canada Permanent resident card?

I landed on Canada in April, 2005 and applied for the permanent resident card. They didn't send me the PR card because they didn't find my address. Now I need to re-apply it. Does anyone know how to re-apply PR card? Do I have to go to Canada again or can my friend in Canada help me re-apply it?
