IMO, don't send a letter like this to immigration office to screw up the officers there. It can not speed up your application, instead, it will add negatives to your application.

Now that they ask you for the second physical exam, it means your application is close to completion. As I know, no applicants have been asked to take third physical exam so far.

You might be under background check in the past two years.
