Help on Notice of Intent to Deny, I485

Hi All:

I just received a Notice of Intent to Deny for my adjustment of Status Case. What INS is asking for now is Employment Authorization Document (I765) to prove that I maintain my status and determine the expiration date of my employment and status as F-1 Student.

The history of my status is as below:

F-1: August 1999 - May 2001
OPT Pending: May 2001 - June 2001
OPT: June 2001 - March 2002
March 2002 to Present: H1B

There is really no gap anywhere in between. I filed my I140 and I485 in July 2005 and I140 has been approved. Then, in August and September 2005, I filed EAD and AP Application and both have been approved.

Before they sent me this notice of intent to deny, they sent me a RFE a few weeks ago asking for all visas/I20/Approval Notices/Cards to show that I maintained my nonimmigrant status since my original entry into the United States. I sent them everything except a copy of the back of the EAD card I applied for Optional Practical Training. After I received this notice of intent to deny, I called National Customer Service Center and they advise me to send both the front and back of the EAD card I had for OPT purpose. Do you guys think that's really the issue?

I am positive that I have never been out of status. But meanwhile, i am a little concerned that I am really missing some documents they are asking for.

I appreciate your help.
