成都父母签证填表疑问，

在成都领事馆的网站上，关于B2签证有下面一条要求：
A completed family biographic data form provided free of charge at one of the designated CITIC Bank branch offices.
这里的Family Biographic data form是什么？158表么？为什么不说158表？
有最近在成都签B2帮助一下。谢谢！
