To Xiaobaitu or any other experts: Question on I-140 form

I have an urgent question on I-140 form. My case is: I first came to US in August 1998. I went back to China for vacation and re-entered US on 01/03/06, and I got an I-94 card from that trip. A week later, my H1B transfer was approved and another I-94 card came together with the approved H1B transfer.

My question is: at the bottom of Part 3, I-140 form:
1: Which number should I fill in for I-94 # - is it the one on the I-94 card I got when I re-entered US on 01/03/06 or is it the one on I-94 when my H1B transfer was approved later?

2. Which date should I fill in for Date of Arrival - is it the date when I re-entered US on 01/03/06 or is it the date when I first came to US back in August 1998?
