关于结婚绿卡。

我要帮我老公办结婚结婚绿卡， 上网站查了一下。

我(公民)需要：file form 1-130 and 2 signed G-325A forms , also need submit the copy of cerficate of Naturalization, photos, marriage certificate

他需要：file form 1-485, from G-325A, from 1-693, form 1-864, and form 1-765.

我有几个问题想请教大家。
1) 我们应该寄这些表格去local INS office, or USINS NEBRASKA SERVICE CENTER. We are in Missouri.
2) 请问去哪里作Medical Examination，大概需要多少钱，因为我老公现在还是学生,没有医疗保险。
3) 以上的表格都需要一起发出去吗？
