help needed for AP

I am trying to apply for AP by myself. While I fill out I-131, I get some questions. Could anybody here give me a hand? I appreciate very much.

1) A#, where can I find this number? on 485 receipt or?
2) Class of Admission, what is this for?
3)Date of Intended departure? I am planning to go back to china next year, However, I am not sure about the exact time yet. So can i put any time?
4)purpose of the trip? Usually what do people put in this question?
5)How many trips do you intend to use this document? again, an exact number or a guess? I plan to go back once next year. As far as I know, AP only lasts for one year and you need to extend it.So the number of trips is for one year?
