Following list provided by my lawyer.
1.Your passprt ( must have a remianing validity of at least 6 monts)
2. Visa processing fee, and reciprocity fee
3. Completed visa application, DS-156 and a two-inch by two-inch photograph
4. DS-157
5. Your original, valid non-immigration visa petition Notice of Action ( Form I-797 Approval Notice ) and I-94 attachment ( bottom portion of the Form I-797 )
6. Photocopy of your most recently H-1B petition.
7. An employment verification letter.
8. Your lastest paycheck stub.
9. Your company's current annual report
10. Copies of your most recent US tax returns, if applicable
11. Your marriage certificate, and children's birth certificates, if applicable
12. Photocopies of all documentation showing lawful status druing all prior stays in the USA, in any classification. This may include copies of your FORMS I-20, Forms IAP-66, Forms I-797, EAC, as well as degrees and trans, if applicable.
