EAD again

First I really appreciate so many well-knowledged people's involvement for offering real information. I am trying to apply EAD. The materials required include: I-94 and last EAD. I have two questions:
1, I have two I-94. One is the one when I entered USA, the other is the one atttached with H1B1. which one should I submit for tthe EAD application? Or both?
2, Is the last EAD the H1B1? Currently, I am using H1B1 and my I-485 is unfortunately pending.
