When shall I start my H1?

I am sorry if this is discussed before.
I am in J1 and going to apply H1. My waiver application was sent to DOS on the early of Oct. I want to apply H1 with premium. When shall I start? If I start now, what happend if the waiver is not ready by then? Are they going to on hold and push DOS or just reject my application?
