help bout fill ETA 750B

I live in Ann Arbor, MI and try to file the I140/485 together.

I tried to fill in the ETA 750B form, in item 10B, which city and state I should use for "aline is in the US and will apply for adjustment of status to that of a lawful permanent resident in the office of the immigration and Naturalization Service at"

Did somebodyknow where I can find it I mean which service center?
